Responsibilities

Position Summary:

The Outpatient Navigator is a Registered Nurse responsible for coordinating and managing the care of complex, High Risk for Readmission patients discharged from Mercy Hospitals. This position will require flexibility. The Outpatient Navigator will receive referrals from Disease Management, make home visits, clinically assess patients and navigate the healthcare system for and with the patient/family. This person works with disciplines both in the hospital and the community to prevent unnecessary readmissions. The Outpatient Navigator is proactive and takes initiative. This position reports to the Director of the Community Wellness Program.

Qualifications

Minimum Requirements:

  • CA RN License

  • BLS (AHA required)

  • Three years experience as a clinical nurse in an acute care setting. One year is defined as 2080 hours for eight hour shifts.

  • Case management or home health experience.

  • Must demonstrate ability to apply technical and critical principles, treatment, and evaluation of patients and families.

  • Must demonstrate professional leadership skills that include problem resolution capabilities.

  • Excellent communication skills and positive attitude with the ability to work well with others as part of a team.

  • Must have working knowledge of computers and microsoft office 97 or better. (word, access, excel, and powerpoint).

  • Must speak fluent English and Spanish.
