The Hospice case manager is responsible to manage hospice patients as part of the hospice team. Some after hours on-call if necessary to cover open shifts when regular scheduled staff is not available.

Give nursing care the way nursing care was meant to be! With the support of the entire hospice team of professional and volunteer care for patients and their loved ones as they face a life limiting illness. Provide competent, compassionate care when it matters most. The incumbent provides care, treatment and service to Hospice patients and their families in their place of residence. Functions as a member of the Interdisciplinary Team.

  • 1 year Hospice nursing experience in the previous 3 years

  • Currently licensed as an R.N. in California

  • Current California driver's license, possession of automobile insurance, and access to an operating motor vehicle

  • Current BLS from the American Heart Association


  • BSN degree

  • Public Health Nurse certificate

  • CHPN certificate
