The Program RN Case Manager is responsible for assessing the physical and functional needs of persons living with HIV/AIDS and developing, implementing, monitoring and evaluating an agreed upon individualized plan of care for each client. Responsible for informing clients about medical and treatment services and coordinating care with medical and non-medical providers. Through intervention and education, the RN Case Manager supports the client in maintaining the highest degree of personal control and independence.


Minimum Requirements:

  • 2 years of experience as an RN in an acute care facility

  • 1 year of HIV/AIDS related clinical experience

  • Current California RN licensure

Preferred Requirements:

  • Experience in home health and/or Nurse Case Management

  • Ability to speak Spanish

  • BSN preferred
