Under the supervision of the Senior Director of Home Health for the Central Coast Service Area, the Chronic Disease Management Program RN works with medical professionals and the patient and their family to plan, organize and arrange necessary services while fostering communications and effective, efficient care. The overall goal is to assure quality of care outcomes and to enhance quality of life as the patient with chronic disease transitions through the medical system. Applies concepts derived from management and communications theory, group dynamics, counseling methodologies, and clinical nursing judgment to implement a clinical nursing program or programs. Manages web-based care management data-base and functions as liaison between the provider, community and community based patients with chronic disease. Primarily provides remote case management via electronic &/or tele-monitoring. Utilizes the nursing process and formulates interventions using current, evidence based clinical knowledge. Initiates and participates in patient, family and staff education. Actively markets the program and participates on pertinent Dignity Health and/or community committees. Demonstrates leadership capabilities. Major responsibilities are listed; however, other duties may be assigned.
Registered Nurse with at least five years of experience in an acute care, critical care, home health or telehealth environment, with an emphasis on care of patients with chronic disease, e.g. CHF, COPD, Diabetes.
California Registered Nurse License.
Current CPR certification and current California drivers license with proof of liability insurance for automobile.
