The Program Coordinator is responsible for the development, coordination, and maintenance of the care continuum. The Program Coordinator provides leadership for all aspects of the continuum of care beginning before surgery through one year post-op; including pre-admission testing, peri-op services, inpatient units, and all other applicable entities within the service line related to evidence-based practice. The Program Coordinator assists with the planning, development, and implementation of co-management goals and program operations. Coordinates and oversees the ongoing disease-specific certification process to continually improve care for patients
Located in Glendale, AZ, this position is critical to the success of St. Joseph's Westgate Medical Center and requires the full understanding and active participation in fulfilling the Mission of Dignity Health.
Minimum of three (3) years acute care experience.
Current AZ RN License in good standing or Compact State.
BLS (American Heart Association).
Five (5) years acute care experience, including experience with general and colorectal surgical patients.
Wound/Ostomy or Bariatric experience
