Reporting to Director of Value Based Operations, this position is responsible for leading activities related to the optimization of performance for Value Based Agreements (capitated lives or through other unique arrangement) for the Greater Sacramento Service Area (GSSA). This position works collaboratively with the Value Based Operations Senior Director, Dignity Health corporate staff, local hospital leadership, local medical group leadership, clinical and operational staff to meet appropriate quality, patient satisfaction and value based agreement goals. The position oversees a mix of clinical, operational, and business activities related to the management of Value Based Agreements. This position shall support overall strategic plans, goals, and direction of quality improvement /process improvement activities.It is expected that the employee will demonstrate behaviors and attitude consistent with Dignity Health core values and humankindness philosophies.
KEY RESPONSIBILITIES & ACCOUNTABILITIES:
Responsible for leading and managing the strategic development, implementation and on-going management of initiatives intended to achieve Value Based Agreement goals
Responsible for managing operational, strategic and financial ROI of initiatives and maximizing efficiencies in execution
Collaborate with Dignity Health Hospitals and GSSA medical group / IPA staff for the purpose of optimizing the delivery of quality and efficient care to identified patient populations for which the GSSA is at risk.
Leads efforts to identify best practices, processes, methodologies for streamlined execution
Develops mitigation plans to address barriers and challenges
Maintain channels of communication between and among entities throughout the GSSA, which encourages interdisciplinary cooperation, respect, and support
Work in partnership with internal Dignity Health and external departments/vendors, when applicable
Coordinates and oversees multi-disciplinary team meetings
Perform other responsibilities and functions as directed
