Responsibilities

Methodist Hospital , a member of Dignity Health, is currently looking for an experienced Pharmacist to add to their hospital staff. The Pharmacist provides timely, accurate, and complete pharmaceutical care, which includes delivery of the appropriate product as well as drug information and drug usage interventions. The ideal candidate will easily demonstrate competency in patient care for neonate, pediatric, adult, and geriatric patients.

Requirements:

  • Pharmacist must have a California Registered Pharmacist License.

  • Requires demonstrated ability to enforce formulary decisions of the medical staff, maintain accurate and complete records, provide instruction and preceptorship on the subject of pharmacotherapeutics, its use and application.

  • Must effectively communicate, both in written form and verbally.

  • Demonstrated knowledge of pharmacotherapeutics, therapeutic drug monitoring and therapeutic interventions, pharmaceutical supplies, drugs, chemicals and biologicals, federal and state regulations relating to pharmaceutical transactions, pharmacokinetics, supply management and inventory control.

The Pharmacist shall demonstrate behaviors consistent with the core (Mercy) values in support with Methodist Hospital and the Mission and Philosophy of Dignity Health.

Qualifications

Acute care in-patient hospital experience preferred. Demonstrated ability to enforce formulary decisions of the medical staff, maintain accurate and complete records, provide instruction and preceptorship on the subject of pharmacotherapeutics, its use and application. Effectively communicate, both written and verbal. Demonstrated knowledge of pharmaco-therapeutics, therapeutic drug monitoring and therapeutic interventions, pharmaceutical supplies, drugs, chemicals and biologicals, federal and state regulations relating to pharmaceutical transactions, pharmacokinetics, supply management and inventory control. Current California pharmacist license.

BCPS preferred. Transitional care pharmacist duties, and other clinical duties. Focus will be transitional care which includes but not limited to: -Discharge medication education for patients. -Medication reconciliation at admission and discharge. -Medication related post discharge issues.
