The Palliative Care Coordinator is responsible for planning, developing, implementing, coordinating, and evaluating palliative care services in a manner consistent with the mission of Community Hospital of San Bernardino. The coordinator will also be responsible for the developement and maintenance of strong relationships with physicians, and staff in their efforts to deliver palliative/comfort care to the seriously or terminally ill patient. This position is accountable for those functions which enhance the delivery of palliative care through education, standards, and program development, quality improvement, consultation and care mangement. In addition, this position is responsible for developing and maintaining relationships in our community for both referral and resource management (hospice, home health, nursing home, etc) The Palliative Care Coordinator will also work collaboratively with the Case Management team and collaborate with patients, families, phyicians and patienst's needs, maintaining cost effective quality care. This position requires the full understanding and active participation in fulfilling the Mission of Community Hospital of San Bernardino. It is expected that the employee will demonstrate behavior consistent with the Core Values. The employee shall support Community Hospital of San Bernardino strategic plan and the goals and direction of the quality improvement, process, and activities.

REQUIREMENTS: Graduate of an accredited school of nursing with a Bachelors degree in Nursing. Completion of ELNEC course is preferred. Minimum of three (3) years eof clinical nursing experience is required. Three (3) years of nursing experience in Case Managment, Hospice, or Palliative Care is preferred.
