The Ambulatory Care Social Worker functions as a leader of the Dignity Health care interdisciplinary team (IDT) collaborating with all departments to ensure that a comprehensive treatment plan of care is developed, monitored, and amended as necessary, and that appropriate outcome measures are identified that demonstrate attainment of individual patient goals. The Ambulatory Care Social Worker will assess, plan, implement, coordinate, monitor and evaluate options and services to meet a patient's need within an assigned case load, using available resources to assist in achieving an effective treatment plan while promoting quality health care with cost effective outcomes. Special attention to detail in the patient population's psychosocial needs and assistance is a must.
The Ambulatory Care Social Worker works closely with the RN Patient Care Coordinator to coordinate care plans based on the utilization review activities performed by the RN Patient Care Coordinator in compliance of federal, state and third party payers. Will be required to participate in performance improvement initiatives related to appropriate patient care management and perform other departmental duties as assigned. The care team program improves the quality of care and clinical outcomes for members with complex care needs by coordinating care within the health care delivery system, while collaborating with a multi-disciplinary team, flexible, case management services to adults within Dominican Medical Group within continuity of care context, including advocacy for services within the community, social services system.
Three (3) years of experience in a clinical setting performing clinical social work or equivalent combination of education and experience
Master of Social Work (MSW)
Clinical training in an acute care or clinic environment
Demonstrated expertise in medical terminology, disease states
Demonstrated problem solving, collaboration, management skills including coaching and counseling, development/training skills
Must possess excellent oral and written communication skills, including excellent customer service skills
Highly skilled in use of desktop software, including Excel and Word
Three to five (3-5) years of experience post licensure with an emphasis on mental health
Case management experience
Experience working with geriatric population and substance abuse
Preferred chronic disease management training
Clinical training in managed care capitated healthcare environment
Knowledge of evidence based clinical practice guidelines
Professional certification (CCM, CCP) preferred
