Job Summary:

The LVN-Home Health Intake works under the supervision of the Director of Patient Care Services to coordinate, organize, monitor, and complete the Home Health (HH) referral process in collaboration with other members of the healthcare team. Reviews and analyzes information to determine appropriateness of HH services and obtains additional

information as needed. Obtains physician orders and seeks clarification as needed. Promotes HH and educates referral sources and the community about available and covered services. Maintains knowledge of Home Health industry regulatory and coverage criteria. Augments HH marketing services by utilizing interpersonal skills to maximize

customer service in a manner that supports excellence in customer service, safe pateint care, and professionalism. Contributes to a work environment of caring and cooperation among a culturally diverse workforce and patient population. Performs other duties as needed


Minimum Required:

Experience working as a LVN in a direct patient care role with working knowledge of Medicare, Medi-Cal, and third party payers required

Minimum 2 years experience

Completion of a Vocational Nursing Program required Demonstrates excellent customer service skills. Excellent

interpersonal skills and the ability to work with others across all agency departments. Demonstrates a caring, compassionate attitude toward others and is able to work

calmly and respond courteously when under pressure. Demonstrates ability to organize and prioritize work efficiently. Ability to multi task, retain and process referrals

accurately and coordinate referrals with nursing supervisor. Demonstrate clear, effective written and oral English skills, Home health experience preferred Home Health Intake experience preferred ICD-9 Coding experience preferred Knowledge of Home Health payer specific information and authorization process preferred.
