About This Position
Under supervision, the Case Manager plans, organizes and manages a patient caseload: applies concepts derived from management and communications theory, group dynamics, counseling methodologies, and clinical nursing judgment to implement a clinical nursing program or programs.
Provides direct patient care through use of nursing process, performs established nursing intervention utilizing current clinical knowledge. Initiates and participates in patient, family and staff education. Demonstrates leadership capabilities. Serves as preceptor for orientees and students. Major responsibilities are listed; however, other duties may be assigned.
RN Selection Criteria:
At least one year as an RN Home health experience preferred. Completion of Hospice Orientation Program Demonstrates ability to work within a decentralized model of patient-care delivery. Demonstrates ability to work with patients who are in the last stages of a terminal illness and their families. Understands philosophy of hospice and needs of the terminally ill.

Reacts calmly and efficiently in emergencies. Acts as a role model for new orientees/students. Demonstrates ability to work with patients who are in the last stages of a terminal illness and their families. Excellent interpersonal skills and the ability to work with others across all agency departments.Ability to achieve results for the organization with and through people, utilizing creative and innovative solutions and conflict management problem-solving skills. Understands philosophy of hospice and needs of the terminally ill. California Registered Nurse license. Current CPR certification and current California drivers license with proof of liability insurance for automobile.
