Overview

Responsibilities

About This Position

Manages the care system to ensure accurate placement, highest reimbursement and coordinates the plan of care with the physician, MMC and MECC through discharge. The Intake Coordinator accepts and coordinates referrals for MECC. Interfaces with departments and external customers to assure care coordination from prior to the point of admission. Anticipates reimbursement based on knowledge of functional levels of patient. Determines placement within facility related to functional status and needs and room availability. Demonstrates ability to organize and prioritize work efficiency. Demonstrates excellent customer service skills Demonstrates clear, effective written and oral English skills and objective active listening skills. Excellent interpersonal skills and the ability to work with others across all departments. Knowledge of JCAHO, OBRA and Title XVII requirements of long term facilities.

RN Selection Criteria

A minimum of two (2) years acute care and/or long-term care experience within the last two (2) years . A working knowledge of Medicare guidelines and managed-care guidelines desired. An understanding of RAI development desired. Current California Registered Nursing License and current CPR certification
